The forms are mailed and postmarked to students by January 31st and include the following:

Box 1: Total payments received for the calendar/tax year from all sources and applied to qualified tuition and related expenses.
Box 2: Now no longer used, will be grayed out.
Box 4: Adjustments made for a prior year
Box 5: Scholarships or grants
Box 6: Adjustments made to scholarships or grants for a prior year
Box 7: Checked if Box 1 amounts include payments for an academic period beginning January–March of the following calendar year
Box 8: Indicates at least half-time status
Box 9: Graduate student status
